Many women are surprised to find that their sex drive actually increases during pregnancy, particularly during the second trimester. Partners often react very individually when it comes to sex and pregnancy. While some men find the site of their wife's blossoming body a true turn on, others are ambivalent or even a little turned off by pregnancy. It is important that you are open, honest and communicative with your partner about your needs during pregnancy, and try not to take any emotions your partner may be feeling personally.

Remember that pregnancy is often an emotional roller coaster, and your husband or partner may be more concerned that you might react differently, or may be scared of the idea of having a family in general. Some men have a difficult time being intimate with their wives even when they recognize that they will not harm the baby, because they feel another presence is in the room.

That said, many men and women have remarkable sexual relationships throughout their pregnancy. Even if you do not engage in intercourse with your husband, it is important that the two of you work on nurturing your relationship throughout your pregnancy. Foot rubs, kissing, back rubs and holding hands are all excellent ways to share some intimacy without actually engaging in intercourse.

The best way you can ensure that you and your partner remain close during your pregnancy is to check in with your partner on occasion. Inform them of your needs, let them know where you are coming from and what you need or want from them.

Positioning

If you and your partner are interested in maintaining a healthy sexual relationship during pregnancy, then undoubtedly you must be wondering what positions will work best for you as your belly grows and expands. The best thing you can do during your pregnancy is keep an open mind and be creative. Phantom Pregnancy

A phantom, or false, pregnancy, which is otherwise known as a pseudocyesis, is a condition in which a non-pregnant woman has all. The symptoms of pregnancy and firmly believes that she is pregnant. A woman who is suffering from a pseudocyesis will visit her doctor or the antenatal clinic complaining of all the symptoms and minor discomforts of early pregnancy, including absence of her periods, breast enlargement and breast changes, nausea and vomiting, increase of weight and abdominal distension. The clinical diagnosis of early pregnancy is extremely difficult, especially if such a person has a slightly enlarged uterus or is overweight, and it may be virtually impossible to confirm or deny the presence of a pregnancy by clinical examination alone. Since it is not the normal practice to perform pregnancy tests upon everybody who attends an antenatal clinic, she will probably be accepted as being pregnant and booked for confinement. The symptoms of pregnancy will continue and as the woman gains more weight she will become increasingly convinced about the presence of her pregnancy, although the fact that she is suffering from pseudocyesis may gradually become apparent to her.

True false pregnancy is extremely rare and it must not be confused with a woman who, having missed one, or perhaps two periods, has no symptoms of pregnancy but visits her doctor to find out why she has missed them, and to ascertain whether or not she is pregnant. The very small group of women who suffer from pseudocyesis are convinced about their pregnancy. It is a condition that occurs mainly in women in their late thirties or early forties who desperately want a child and have been trying to become pregnant for many years. It can also occur in younger women -especially when relatives and friends are having babies or asking and joking about the girls ability to become pregnant. Women who suffer from false pregnancies are normally emotionally quite stable but may become very unstable over the question of pregnancy.

Phantom pregnancy also occurs in some women who have lost a pregnancy or a baby. It may be an emotional reaction, but, in this case, there is increasing scientific evidence to indicate that a temporary hormone imbalance is the cause of all their symptoms.

The diagnosis of phantom pregnancy is extremely difficult, especially at the beginning of a false pregnancy. The uterus will remain normal in size and successive pregnancy tests, if performed, will be negative. As the false pregnancy proceeds, a marked increase in weight may occur, together with abdominal and breast enlargement, but the uterus will not enlarge nor will the breasts show active signs of pregnancy. None of the other physical signs of pregnancy will develop and all tests for pregnancy will be negative.

A woman who is suffering from true pseudocyesis will insist that she is pregnant despite the assurance that she is not. The problem requires very careful and sympathetic consideration with kindly explanation to the patient as to the reasons why she is not pregnant, together with appropriate proof that a pregnancy does not exist, although she may find even this difficult to accept.

Severe emotional disturbance may follow the realization that she is suffering from a false pregnancy and the woman's husband should also be acquainted with all the facts and treated with complete confidence, because his help is invaluable.

In past years this was a relatively confusing condition but with modern ultrasound scanning being available in most maternity units the diagnosis of pregnancy is usually made or refuted at a relatively early stage of   rel=nofollow pregnancy. Advanced phantom pregnancy is therefore an uncommon condition these days.

Pregnancy Antenatal Care - Your Health Care Team and Appointments

During pregnancy, yours and your baby's health is monitored closely, but who should you see? When do you need to see them? And what are all the pregnancy tests and scans for? Finding out what antenatal care you should expect when you're pregnant can be confusing, so here's our guide to help you through this exciting time.

Your Pregnancy Health Care Team

Your team of health care professionals during pregnancy can be quite varied. Initially you should see your GP (family doctor) when you think you're pregnant and they will help you plan your antenatal care.

Your doctor should tell you how to arrange your first appointment with your midwife (otherwise known as your 'booking in appointment') and from there the midwife will look after you and your baby throughout your pregnancy and for the first few weeks after your baby is born.

You may also have routine appointments with an Obstetrician at the beginning and end of your pregnancy. They specialise in pregnancy and birth and will see you more regularly if you are carrying multiple births, have any complications with your pregnancy or are having a planned cesarean.

When your baby is a few months old, your Health Visitor will take over the care of you and your baby from your midwife. They are likely to visit you to see how you're getting on and are available for any worries or concerns you may have as your baby grows.

Pregnancy check ups

During your first pregnancy you are likely to have up to 10 appointments to attend to. This reduces to around 7 in subsequent pregnancies.

The number of check ups you have when pregnant depends on many things:


Your risk factors
Any complications
Elected labour options
The number of babies you are carrying
The area in which you live.


All pregnant women will be given their medical records to keep hold of during pregnancy. Don't forget to bring them to all of your appointments so that whichever doctor or midwife is treating you will have your notes to hand.

Booking in appointment

The first meeting with your midwife will happen when you are around 8-10 weeks pregnant and is referred to as the 'booking in' appointment.

This is when your midwife will discuss your family history (and your partner's), give you advice about your diet, lifestyle and staying healthy whilst you're pregnant.

The midwife will also carry out some tests such as urine and blood tests, you'll have your blood pressure checked and some details about your body recorded such as height, weight and tummy measurement. This is all for your midwife to monitor the health of you and your baby.

Dating 12 week scan

Many clinics offer a scan at between 10 and 14 weeks. Pregnancy blood tests

You should have a range of blood tests in your 2nd trimester, usually between the 14th and 20th week of your pregnancy. These are routine and are offered to all pregnant women:


Blood group tests identify whether you are Rhesus Negative (RhD). This can cause complications in later pregnancies, but simple injections during your first pregnancy will prevent this.
You'll have what's called a full blood count test (repeated again at 28 weeks) which helps to monitor you for deficiency anaemia, a common complaint during pregnancy.
You'll also be screened for infections and viruses such as Hepatitis B, syphilis and HIV. This is a precautionary measure and just means they can look after your baby to prevent you passing the infection on during pregnancy.

The 20 week anomaly scan

Around your 20th week of pregnancy, you will have an anomaly scan. This is a detailed check on your baby using ultrasound and examines the baby's body, measurements and growth. They'll also check on your placenta and you may even have the option of finding out the sex of your baby, although not all places offer this and no hospital guarantees the sex from the ultrasound.

You can usually ask for a picture from this pregnancy scan, but again expect to pay a charge. Some places even offer a video.

Additional tests and scans

Other than the standard pregnancy scans and tests you may be offered additional checks, either as optional tests if you are at risk of any complications, or as extra monitoring of your baby. The important thing to remember is to discuss any concerns you have with your midwife or doctor, they are there to help make your pregnancy as healthy, safe and enjoyable as possible.

Exercising and Pregnancy - Keeping You and Your Baby Safe

The best time to start planning your health, weight and exercise program is the same time as you are starting to plan your pregnancy; having a strong, fit and healthy body will not only prepare you for the strength and stamina required during your pregnancy, but it will also increase your chances of conception and an easier pregnancy, labour and birth!

The other reason to start your health, weight and exercise program prior to conception, is that pregnancy is no time to start anything new as it may cause unwanted stress to you and your baby. Any activity you are doing in the 6 months leading up to your pregnancy is generally ok to continue through the term of your pregnancy, however a visit to your Doctor or Obstetrician is always recommended to discuss the appropriateness of your activities and the intensity at which you should be training through the different stages of your pregnancy.

Strength training is now recommended by the American College of Obstetricians and has been associated with:


Making pregnancy easier
  Helping avoid weight gain
  Decreasing time in labour
  Making labour easier
  Quicker recovery after birth
  Reduces tiredness
  Better control over body-fat fluctuations
  Increased strength to perform daily activities during and after pregnancy

Exercising during pregnancy

Personal Training and pregnancy

There are many advantages to using a personal trainer as they will be aware of the safety considerations for you and your baby as well as correct technique, nutrition, suitable and practical exercise for the preparation of giving birth and of course, support and encouragement!

In particular, the benefits of having a personal trainer is to start a supervised strength training program in the six months leading up to your planned pregnancy (and from my experience, healthy clients who plan for pregnancy certainly fall pregnant quickly).

Safety considerations for exercise during pregnancy

To ensure that your exercise program provides you with all of the wonderful benefits outlined above, you will also need to ensure your safety and that of your baby, so below is a list of safety considerations to be aware of; and remember to always speak to your Doctor or Obstetrician if you are unsure.

1. Avoid Overheating. Your growing baby does not have the same ability to dissipate heat as you do. To avoid overheating:


Avoid prolonged exercise.
  Stay well hydrated.
  Do not use sweating as an indicator of how hot you may be getting.
  Avoid exercising on hot, humid days. Use fans during hot weather.
  Wear light, loose fitting clothing. Cotton is best.


2. Avoid saunas and steam baths at all times during pregnancy.

3. Avoid high intensity exercise. Studies have indicated that when a mother's heart rate stays in a range of approximately 140bpm, the foetus has no abnormal responses. To keep exercise intensity at a safe level, follow these guidelines:


Change from an intermediate or advanced aerobics program to a lighter paced program.
    Learn how to measure your heart rate and check it regularly.
    Be realistic about the need to exercise in moderation.
    Have a prolonged cool-down after the aerobic portion of the workout.


4. Avoid frequent and prolonged exercise after week 28 of your pregnancy.

5. Limit the amount of exercise that you do lying on your back. This is of particular concern from your second trimester on.

6. Avoid the use of hand weights over 0.5kg in weight during aerobic classes.

7. Perform Pelvic Floor (Kegel) Exercises.

8. Wear a good supportive bra.

9. Avoid rapid changes in direction and be very cautious if you are doing Step exercise classes.

10. Stretch gently.

11. Have a light snack approximately 2 hours before exercise and carry a small carton of fruit juice to your workout.

12. Stop exercise: if at any time during your exercise session you feel very hot, faint, dizzy, short of breath, experience vaginal bleeding, have palpitations, blurred vision, or severe or continuous headaches - stop immediately . It is also important to stop if you experience lower abdominal pain, tightness or cramping, back pain or pubic pain. If you experience any of these symptoms, consult your obstetrician.

Neck Pain, Back Pain, Sciatica and Pregnancy - The Back Pain Complex, Causes and Treatment

The back pain complex, to include neck pain, back pain and sciatica, is one of the most common complaints among mothers-to-be during pregnancy. The spine is made up of two primary and two secondary curvatures. The secondary curvature of the lumbar region is most affected during pregnancy. The lumbar curvature, established after birth and responsible for our ability to walk upright, comes under tremendous pressure during pregnancy. Back pain is quite common during pregnancy with as many as 80% of all women experiencing neck pain, back pain and/or sciatica in varying degrees. The back pain complex is expressed from mild to acute and may become chronic if not properly addressed. We will discuss some of the reasons why back pain is so prevalent during pregnancy and offer a few solutions, as well.

While women with previous back problems are at greater risk for neck pain, back pain and sciatica, it is common among all women beginning as early as two months and continuing through, and well after, the completion of the pregnancy. During pregnancy the body produces a number of hormones, some in much higher quantities than typically found. One of the principal hormones having the most dramatic impact during pregnancy is called Relaxin. Relaxin's function it is to make the ligaments associated with birth, and the pelvis aperture itself, softer and more pliable. The reasons for this are obvious, it is vitally important for the baby, particularly the baby's cranium or skull, to be able to pass through the birth canal with relative ease, Relaxin makes this possible. Additionally, as the uterus grows and weight shifts forward, the center of gravity changes and the relaxation of the uterine ligaments impacts the spine and the surrounding musculature. The core muscles, to include key abdominal muscles, spinal muscles, and para-vertebral muscles, are also stretched and taxed beyond normal capabilities. The combination of hormonal and structural factors often contribute to varying degrees of neck pain, back pain, and sciatica.

Obesity, always a significant factor in diagnosing and treating the back pain complex, becomes an even greater issue during pregnancy. Normal weight gain, not to be confused with obesity, may also negatively affect mothers-to-be. The combination of an exaggerated spinal curvature, called lordosis in the lumbar region, a shift in the center of gravity, additional weight carried forward, and a weaker core muscle complex are also contributory factors. The core muscles, as well as other synergistic muscle groups, may be strengthened through a program of exercise and stretching. It must be recognized that back pain during pregnancy is the rule rather than the exception, the challenge is to deal effectively with the underlying cause or causes with an intelligent and individualized treatment program.

Two of the most common back pain expressions during pregnancy, are lumbar pain, centered primarily in the lower back, and posterior pelvic pain, with symptoms very much like those associated with sciatica. The low back or lumbar pain is generally centered in the lower back and may radiate unilaterally, one side, or bilaterally, both sides, across the iliac crest, which is the region to each side of the spine at the upper level of the pelvis. Once again, pain may range from mild to acute and grow increasingly intense the deeper into the pregnancy the mother-to-be is. Back pain in this region is exacerbated by extended periods of standing, particularly in one place, sitting for long periods of time, and by poor posture, as in the old military idea of "stomach and chest out" which simply does not work for obvious reasons. Posterior pelvic pain can be quite severe and may be four to five times more prevalent than any other back pain expression during pregnancy. Posterior pelvic pain, as noted above, may be indistinguishable in its expression from sciatica. Pelvic pain may be felt as a deep ache or even a burning sensation, may radiate across the buttocks and into the legs, unilaterally or bilaterally, and in its worst configuration or state may also be felt in the feet and toes, very much like sciatica. The biggest distinction between posterior pelvic pain and sciatica is that posterior pelvic pain is more commonly experienced bilaterally. Sciatica, on the other hand, is generally a unilateral expression affecting the buttocks, legs, feet, and toes.

While quite common, neck pain, back pain and/or sciatica, including posterior pelvic pain, should not be taken lightly. If the back pain complex is not addressed properly, it may have negative consequences throughout the pregnancy and felt in virtually every aspect of the new mom's life. It is not uncommon for women who experience severe back pain throughout the pregnancy to continue to have problems well into the post partum period. Pregnancy can be, and usually is, a wonderful time in both the family's life and in the life of the mother-to-be. However, if back pain is not addressed it may have a negative impact during and afterwards, in some cases for years after the pregnancy.

As noted above, there may be a myriad of causes and consequences associated with the back pain complex, as it relates to pregnancy. An intelligent, individualized, and medically supervised program of exercise and stretching is not only advisable it is absolutely necessary. Certain exercises, such as the pelvic tilt and crunches, at least until the latter stages of the pregnancy, may be appropriate and advisable. However, it is usually the case that such self-treatment programs of exercise are not only misguided but fail to achieve any real and lasting relief or benefit. Additionally, because of the shift in the center of gravity and the exacerbated lumbar curvature, a complete program focusing on the core muscles may be the best approach. Improper posture, standing for long periods of time, sitting for long periods, and even remaining inactive for extended periods of time may have a negative impact, ultimately leading to even greater pain. Finally, it is advisable for pregnant women, particularly those who work deep into the pregnancy or have other children, to take special care when lifting anything heavy. Because of the number of physiological and anatomical changes that take place during pregnancy, a program that seeks to strengthen the musculature and to improve endurance, so fatigue doesn't lead to other problems and injuries, should be initiated as soon as possible. When Does the Pregnancy Test Become Positive?

For a pregnancy test to become positive, the fertilized egg has to first implant into the uterus and then send enough amounts of hCG the pregnancy hormone into your blood stream for enough hCG to create a positive pregnancy test.

Implantation, when the fertilized egg implants into the endometrium, happens about a week after ovulation (range: 6-12 days), and it takes another 3-4 days after implantation (9-10 dpo: days after ovulation) for the blood pregnancy test to first become positive.



Blood Pregnancy test: If you are pregnant then the blood pregnancy test is usually positive within 3-4 days after implantation or about 9-10 days after fertilization and ovulation.
Urine (Home HPT) Pregnancy Test: The urine pregnancy test will usually not become positive in most women until a missed period or about a week after implantation. Many manufacturers of urine pregnancy tests want you to believe that the urine test is usually positive before a missed period, but only about 25% of pregnant women will test positive with the HPT 2 days before a missed period, and about 40% the day before the missed period.  The wide variation when the urine pregnancy test becomes positive is due to four variables which determine when the home urine pregnancy test becomes positive:

1. Timing of Implantation

A pregnancy test detects the presence of the pregnancy hormone hCG (human chorionic gonadotropin) either in the blood or in the urine. The hCG in the urine comes from the hCG in the blood after it was filtered through the kidney. After ovulation, the egg gets fertilized and travels through the fallopian tube toward the uterus. This travel takes on average about a week. Implantation, the attachment of the fertilized egg in the lining of the uterus, occurs on average about a week or later after ovulation/fertilization, but it can happen as early as six days and as late as 12 days after fertilization/ovulation. At the time of implantation, the placenta starts making the pregnancy hormone hCG, which then enters the blood stream. hCG can be detected in the blood about three to four days after implantation. Urine hCG can be detected about two to three days after blood hCG can first be seen. Detection of hCG depends on timing of implantation, so hCG can normally be detected in the blood between nine and 16 days after ovulation (HPT: 12-19 days after ovulation).

2. Blood hCG Level

There is a wide normal variation of hCG levels, both in the blood and the urine. If a woman has low blood hCG levels, for example, then it can take several more days for it to show up on a pregnancy test.

3. Urine hCG Level

Urine hCG levels change over the course of the day depending on how much you drink and how diluted or concentrated the urine is. Urine pregnancy testing is best done with more concentrated urine. If you drink a lot of fluid, the urine may be too diluted early on in pregnancy to achieve an hCG concentration adequate enough for a positive test.

4. Sensitivity of Urine Home Pregnancy Test

Different pregnancy tests have different sensitivities. The lower the sensitivity, the earlier a pregnancy test becomes positive. Sensitivities are indicated in mIU/mL, the lowest amount of hCG in the urine that tests positive. A sensitivity of 20 mIU/mL requires one-half the hCG level to be positive when compared with a 40 mIU/mL sensitivity level, and it may become positive several days earlier than the pregnancy test with a higher sensitivity.
